Acknowledgment and Assumption of Risk:

I hereby acknowledge that I am voluntarily participating in the Pickleball tournament organized by the GAMA. I understand that participation in any sports activity involves physical exertion and carries inherent risks, including but not limited to, the risk of injury, property damage, or in rare cases, serious medical conditions.

Waiver and Release of Liability:

In consideration for being allowed to participate in the above event, I hereby release and discharge GAMA, its officers, committee members, volunteers, event organizers, and affiliates from any and all liability, claims, or demands arising out of or relating to any injury, loss, or damage that may occur as a result of my participation in this event, whether caused by negligence or otherwise.

Medical Consent:

I certify that I am physically fit and able to participate in the activities involved. In the event of a medical emergency, I authorize event staff to seek medical assistance on my behalf.

Photo/Video Consent:

I grant permission for GAMA to use any photographs or videos taken during the event for promotional purposes, including on social media, newsletters, and websites.

Special Note for Minors (Ages 13–17):

Participants must be 13 years of age or older to take part in this event.

If the participant is a minor (under 18 years of age), a parent or legal guardian must acknowledge this waiver online on behalf of the minor by completing the section below. Typing the parent/guardian’s name will serve as a digital signature and indicate acceptance of the waiver terms.
